The Health Professions Act, RSA 2000 cH-7 s33 (3-4) and the Occupational Therapists Profession Regulation (AR217/2006) s35 requires that regulated members must provide personal and employment information upon application and immediately when there is a change to that information.

Employment Change:

If you have had a change in employment you can change your employment details by logging into the members section on the ACOT website. Please note that if you have changed employers you must put an end date on your previous employment before the system will let you add a new employer.

Change of Name:

A change of name requires completion of a Change of Information form and proof of change of name (marriage certificate, drivers license, etc.) The completed form can be faxed to F: 780.434.0658, emailed to info@acot.ca or mailed to Alberta College of Occupational Therapists, 312, 8925-51 Avenue, Edmonton, AB T6E 5J3.
